Chellaram Hospital – Diabetes Care & Multispeciality (referred to as “we”, us”, Chellaram Hospital – Diabetes Care & Multispeciality) is the author and publisher of the internet resource www.cdi.org.in on the world wide web as well as other software provided by Chellaram Hospital – Diabetes Care & Multispeciality, including but not limited to the websites referred to as “Services”). Chellaram Hospital – Diabetes Care & Multispeciality provides the Services in partnership with its agents, affiliates, associates, representatives or other third parties (together referred to as “Partners”).

Why this privacy policy?

This Privacy Policy is published in compliance with, inter alia: a) Section 43A of the Information Technology Act, 2000; b) Regulation 4 of the Information Technology (Reasonable Security Practices and Procedures and Sensitive Personal Information) Rules, 2011 (the “SPI Rules”); and c) Regulation 3(1) of the Information Technology (Intermediaries Guidelines) Rules, 2011.

This Privacy Policy states the following:

  1. The type of information collected from the Users, including sensitive personal data or information;
  2. The purpose, means and modes of usage of such information; and
  3. How and to whom we will disclose such information.

This privacy policy sets out how we, Chellaram Hospital – Diabetes Care & Multispeciality, collect, use and disclose your data.

  • “Personal Data” means data (such as name, email address, mobile numbers, government-authorised identity card information, financial and bank account details, health records), about an individual who can be identified from that data or can be linked with such other data and information to identify that individual.

We will share your data with our internal third parties such as front desk, IT department and Billing department in matters pertaining but not limited to appointments, payment and enquiry of services provided by Chellaram Hospital – Diabetes Care & Multispeciality. This would also include information sought by the user from the information Chatbot. The user may expect a telephonic conversation from the front desk in case of enquiry using the Chatbot, within 24 hours of enquiry.

We are the sole owners of the information collected on this website. We will not sell, rent or otherwise disclose your personal information to any third party, without your consent except in the following circumstances:

We will disclose your personal information to any appropriate investigative third party, if we believe that you are involved in any illegal or harmful conduct or if we are required to do so by law or believe that such action is necessary to: (1) comply with the law or with legal process; (2) protect and defend our rights and property or that of our customers; (3) prevent fraud; (4) protect against abuse, misuse or unauthorized use of our website; or (5) protect the personal safety or property of our customers or the public.

Terms & conditions for payment of services

You agree, understand and confirm that the debit/credit card details provided by you for availing of services on www.cdi.org.in will be correct and accurate and you shall not use the debit/credit card which is not lawfully owned by you.

You further agree and undertake to provide the correct and valid debit/credit card details to www.cdi.org.in. The said information will not be utilized and shared by www.cdi.org.in with any of the third parties unless required by law, regulation or court order.

Termination of service due to non-compliance with the agreement

We will use our best efforts to make all your payments properly. However, we shall not incur any liability and any Service Guarantee shall be void because of the existence of any one or more of the following circumstances:

  • If, you have not provided the hospital with correct M R Number, IP number, patient’s name, address, telephone number, mobile number, date of Admission, Service Type, Payment Account information, or the Account information for the payee.
  • If, through no fault of the hospital, your Payment Account does not contain sufficient funds to complete the transaction or the transaction would exceed the credit limit of your overdraft amount.
  • The payment processing centre is not working properly and you know or have been advised by the hospital about the malfunction before you execute the transaction.
  • Circumstances beyond the control of the hospital (Such as, but not limited to, fire, flood, or interference from the outside force) prevent the proper execution of the transaction and the hospital has taken reasonable precautions to avoid those circumstances and/or,
  • In case of payment through debit/credit card, if the details provided by the user are inaccurate, incorrect or entered fraudulently.
  • If the user is in breach of any of the terms and conditions of this Agreement and/or the Terms and conditions of the usage of www.cdi.org.in

Cease payment requests

We cannot assist with a stop payment request for any Bill Payment scheduled using a Payment Account through debit/credit card. In any such case; you must contact your debit/credit card company directly.

Notification

We will use our best efforts to present all of your electronic bills promptly. In addition to notification and acknowledgement after completion of the transaction, details will be communicated to you via email on the email id provided by you.

Password and security

You agree not to give or make available your password or other means to access your account to any unauthorised individuals. You are responsible for all payments you authorize using the services at the hospital website. If you permit other persons to use a password or other means to access your account, you are responsible for any transaction they authorise.

Change and reiteration

We reserve the right to amend our privacy/security policy and make the new provisions effective for all information we maintain from time to time. The revised policy will be posted in our facilities and offices and will be available while obtaining the treatment from our hospital directly. Your use of our website shall comprise of acceptance of any changes of this Privacy and Security Policy.

Other information

Information is supplied upon the condition that the persons receiving the same will make their determination as to its suitability for their purposes before use or in connection with the making of any decision. Any use of this website or the Information is at your own risk. Chellaram Hospital – Diabetes Care & Multispeciality or their officers, employees or agents shall not be liable for any loss, damage or expense arising out of any access to, use of this website or the Information, or any website linked to this website.

Nothing contained herein is to be construed as a recommendation to use any service, product, process, equipment or formulation, in conflict with any patent, or otherwise and we make no representation or warranty, express or implied that the use thereof will not infringe any patent, or otherwise.
